Hi. I know the major scale in 5 positions, sometimes have to refresh by looking at a diagram ...but how do I learn to combine them in a given key and continue up the neck?

have a look at the Scale finder in the Toolbox section.

Gives the patterns for each scale in each key.

Otherwise just do a bit of noodling building a scale along the string using the whole , whole half etc and deciding when to go up a string.

Then we have the Caged system to learn when we become a bit more advanced

Christopher Schlegal does some tutorials on scales, try a search.

This collection of tutorials covers how to improvise. But it is essentially about what you are asking: how to combine scales & chords in a key to play melodically.

You might also find these tutorials on the various patterns of the major scale.

Major Scale Patterns 2

Hope that helps! Please ask more if necessary!
