This makes sense now. The tab I have (Welcome Home from Metallica) has the notation I posted noted as a triad in the tab notation description, hence my original question, since this did not coincide with what I assumed a triad was. I finally purchased a book with a better tab in it and it has it listed exactly as I thought it was to be played: fast with a hammer-on/pull-off combo.
