Check to see if there are any fingers that you can leave where they are when switching between chords. When going from D to G, for example, you can leave your ring finger down (x, x, 0, 2, 3, 2 -----> 3, 2, 0, 0, 3, 3 -- See how they have the 3 on the B string in common?). Take the chord change super slowly at first to make sure you're getting the right motion down, then speed up (practice changing with a metronome. C chord, click, click, click, D chord, click, click, click, E chord, etc.).
