Answers:
one: yes, but it doesn't have to be always the 5th frett, you can try 7th , 3rd , etc. but you will find it hard some places on the frettboard and easier on other pots.. also it doesn't have to be the G string, you may like to try the B string and other strings as well for different pitches, but on high e string it will be very damn hard to do it and keep muting right.
two: I didn't understand this one.. what other notes do you mean? Harmonized notes? if yes, then it's the same thing but you will need a harmonizer effect for that, it's found in most multi-effect unit but to get the right one you'll have to save money for an Eventide.. I have never actualy tried it (hope i could someday) but I know that every guitarist on the planet who can afford it have this unit in his Rack..
By the way, an Eventide DSP7000 Harmonizer costs over $3000
Also as 'MikeP' said, you'll need good equipment to do the scream, not as good dimebag's rig of course, but the most important thing is a very high distortion sound, I can do this trick on any guitar (equiped with tremolo bar) plugged into any amp, but with a Good distortion pedal like the Boss Metal Zone or the DOD Death Metal...
