I hold the pick so that it follows the line of my forefinger, protruding slightly beyond my finger tip and resting on the edge of my finger and my thumb is at 90% to the pick and finger clamped over the widest part of the pick. (i hope that makes sense ) :-)

I've actually seen a lot of people with this problem and I always recommend Sticky Picks by Guitar Moose.
I've played for 20+ years and I use them exclusively. It almost always solves the "picking dropping syndrome."
