OK, so you are already on changing between open chords (A, D, E).

Did everything up to that point go OK?

Is your guitar setup properly? Are the strings low enough to the frets that it is easy to hold them down and play?

Assuming all else is well, I am guessing that most of your problem is getting from one chord to the next in time. That is a hard skill. You need to slow down and be patient with yourself.

You've only been playing guitar for a week, for goodness sake! :)

Also, you mention practicing 5 hours a day. Exactly what are you doing for five hours?!
