E----7----
B----5---- sounds good

E----18--
B----16-- pitch is way off

What can i do to fix this and prevent this?

Take your guitar in to a tech and tell them that you need your intonation set.

I would explain how to set it to you but it takes a little while... Use the search function here on the site and search "intonation" you should come up with a bunch of info... But take it to a guitar tech and get a setup and you'll be fine.
