I think it depends on your own personal preference. Personally I hold mine with the thumb and first finger and don't anchor anywhere and I hold the pick sideways and use the edge instead of the tip. It changes constantly depending on the song and the dynamics you're using though. Same thing with the pick; if I'm strumming I'll switch to a lighter, large pick but if I'm playing fast leads I use a small hard one.
