But it depends on how much dexterity you can manage without one. And maybe how much sensation you have in the tip of you rmiddle finger. Can you play at all? Have you started? If so, does fretting with your middle finger hurt at all?

Also, what kind of music do you want to play? And what kind of guitar do you have? Using lighter gauge strings might help.
