Id say what ever is most comfortable for you. everyone has there own way of holding the guitar. if i were gonna do it id hold my index finger on the 12th fret and use the pinky to hammer on the 17. personally my hand stays pretty vertical when i play but i have small hands and i cant get much reach if i tilt my hand.

thats just my 2 cents but what do i know.
