Thats a 3-screw neck. The serial number indicates it to be an american neck from 1979... The larger hole is for the neck tilt adjustment system. You can drill the other four holes where they need to be... but the value of the neck would go down considerably... if you haven't drilled the holes on your body i would suggest going with the 3-screw configuration and buying a 3-screw fender neckplate... do some research on the 3-screw necks and look around and see what options you have.
