Well, if there's one thing that I wish there was more of in this world, it's pure honesty. But then, I am a total hypocrite there, since, in spite of my desire for honesty, I am also cursed with cowardice.

So, here is "what I would do", but keep in mind that I would probably never actually do it.

For me, I can basically tell if a girl is interested in me by the way she looks at me or the way she acts when we talk. At least, I can tell if I have a fighting chance. If she looks you right in the eye(s) when you're talking, plays with her hair, laughs a lot, there's basically no question there. But if she is pretty quick in her conversations with you and won't look you in the eye, you've got some work to do. Also take note of how she acts around other guys, if she's nice to everyone or just to you.

OK, on to the advice. I actually think that, since you're a bit shy to ask her out in front of a waiting room full of bored people who would love to have some entertainment like a bloke like you getting totally rejected, the note idea may have some merit. Maybe just something like "When's your lunch break, I'd like to get to know you better", or "What time do you get off, I know this great English restaurant (<--- blatant mockery)". Slide her the note when you arrive, then sit down in a chair where you can make some eye contact with her afterwards.

Oh, and BTW, how old are you?
