Probably tapped. When it says to "hammer on" the 20th fret, do it with the index finger of your right hand.

or you could use the edge of the pick (a popular way to do it) or a lot of people use their middle finger.

then you can hold you're pick with your index and thumb

Yeah, but then it loses a lot of the tone and the rhythm is harder to control.

i was just saying a lot of professionals (e.g. Kirk Hammett) uses the edge of their pick

what I was asking for was that, is there like a trick or a better way to play that or anything like that?

I know i'm suppost hit the string on the 18th fret and than hammer on 20, but what I'm curious about is, When I've hammered on the 20th fret am I suppose to hit the 15th string and than hammer on 18? thats what I don't get...
